Job Description
Job Title: Senior CHC Nurse Assessor – Children & Young People (CYP)
Band: Band 6
Department: Continuing Care and Learning Disability Team
Location: Flexible, based across the organization
Reports to: All Age Continuing Health Care Lead, Young Adults Lead
Accountable to: Head of Continuing Health Care and Professional Development
Job Summary:
As a Senior CHC Nurse Assessor, you will play a critical role in facilitating multi-disciplinary assessments and reviews for children and young people’s Continuing Care. You will ensure compliance with national guidelines, the NMC Code of Professional Conduct, and organization policies. You will lead care reviews, coordinate team meetings, manage complex cases, and support transitions for children and young people with continuing care needs. This role requires collaboration with multiple stakeholders, including health, social care, education providers, and families.
Key Responsibilities:
- Lead and facilitate multi-disciplinary assessments and reviews for children and young people in line with national frameworks and policies.
- Manage ongoing case reviews, ensuring seamless transitions from children’s to adult services.
- Collaborate with a range of professionals to create and review care plans and transition arrangements for SEND and other special care needs.
- Support the implementation of Personal Health Budgets (PHB) and ensure safeguarding protocols are met.
- Participate in SEND transition meetings to ensure consistency and clarity in care pathways.
- Commission care packages in line with financial guidelines.
- Provide clinical leadership and specialist advice in the assessment and management of children and young people’s continuing care needs.
Required Skills & Qualifications:
- Registered Nurse with valid NMC registration.
- Experience in Continuing Care assessments, SEND transitions, and complex case management.
- Knowledge of Personal Health Budgets, DOLS, Liberty Protection Safeguards, and safeguarding frameworks.
- Strong communication and coordination skills to work effectively with multi-agency teams and families.
- Ability to manage complex and sensitive cases, ensuring high-quality care and adherence to safety standards.
